A custom booklet combines your policies and procedures with state-specific regulations and accrediting body information (if applicable) into a professionally formatted booklet detailed to meet your agency’s needs. Booklet cost is based on the number of pages in the booklet; quantity ordered, upcharges such as carbonless 2-part NCR forms, front/back cover ink colors, etc. The average cost per book is between $4.00 - $6.00. As the purpose of the booklet is to simplify the admission process and minimize costs, we will advise you during the drafting process where information may be streamlined and ways to keep the cost of the booklet to a minimum. An exact cost per book will be provided to you once the custom booklet is finalized.
A set-up fee will be associated with orders under 5,000 and due prior to development.
Our “Startup/Small Company Package” applies to *home care agencies admitting 15 patients or less per month. The package cost is $795.00 and includes development of your custom booklet as well as your first 25 booklets to get started with (shipping is included). *We have a morotorium on Small/Start-up agencies in the state of Michigan at this time.
No. MedForms begins the customization process by using our master template. Once we receive your agency specific information, our Development Team will work with you on incorporating your materials into an all-inclusive booklet. Our Development Team has extensive knowledge of clinical and administrative aspects of the home care industry and works closely with you during the customization process.
All books are custom and can be designed to meet each individual agency’s requested specifications; however, we have thoughtfully formatted our master templates to be easily understood by both patient and clinician. With content that is written on a 5th - 6th grade reading level, we purposefully format booklets in large fonts, allowing for readability and comprehension. For agencies with corporate branding requirements (font type and size), we adjust formatting to meet marketing style guidelines.
Yes. While MedForms does not provide consultant services, our Regulatory Review Team researches and confirms home care regulations on a routine basis, allowing MedForms to offer our clients suggested changes as state, federal and accrediting organization rules/standards change. We also update the booklets based on surveyor trends as clients report feedback from their accrediting and/or state surveys. While the ultimate responsibility of booklet compliance remains with the agency, we provide this valuable service to our clients free of charge.
We regularly review and update our master verbiage to incorporate the latest Joint Commission, ACHC and CHAP accrediting body standards based on our interpretation of their application in the patient orientation booklets. If you are seeking accreditation, please specify the accrediting body during the development phase. We will review your booklet and incorporate changes as we feel necessary; however, as we are not a consultant, we do also ask that you review the standards and your booklet to ensure compliance.
MedForms does not charge for revisions; a huge cost savings for our clients as changes to state, federal and accrediting organization rules/standards change regularly. MedForms has a knowledgeable and efficient staff of editors who make all changes to existing booklets during the reorder process. When you reorder your booklets, our editorial staff will make any changes you request and also offer our suggested regulatory changes at that time, if applicable.
We work with clients ranging in size from small, one-location companies to large national, multi-state, multi-location agencies. We have many different options for multi-location agencies including printing separate covers (with content of the booklet being shared so that larger quantity price breaks are still available) or by listing each location on the cover of the booklet and adding check boxes for the patient to reference. These needs will be addressed during the development process.
From the time that we receive your written approval of any revisions, it takes approximately 2-3 weeks for books to arrive. For clients in Hawaii and Alaska, please allow an additional week of transit time.
No. Duplicating copyrighted material is strictly prohibited. Our Patient Orientation Booklet is considered a unique and valuable asset of our company and is protected by U.S. Copyright laws under Title 17.
